-Well visits and sports physicals at no additional cost (per AAP schedule)

-Specialized rates for:

• Same day/next day sick visits: $50
• Wellness services (lactation counseling, mental health counseling and more)
• Direct access to Providers

*Subject to additional cost based on in-office procedures/testing or medications

All new patients will pay one-time enrollment fee of $100 

*** This fee is waived for previous patients of Toni Evelyn who are transitioning to her DPC practice.
